My two cents.
Put the input shaft and constant mesh drive gear back in the hole. Snug them in. See just how far the fifth gear slides forward into mesh with the splines. if most of its travel is used up befor it engages, the splines (both internal and external are worn from agressive shifting into high. I think there should be about 3/16" of sliding into engagement.

Seems to me that the gear doesn't look terribly bad, I've seen a lot worse that stayed in gear. I would check the shifter forks, centering balls and springs and even the gear shift lever bottom where it goes into the rail for excessive wear to make sure the rail is moving as far as it should.

Levi,
Several years ago my father had an old worn out H that always jumped out of 5th gear. An old IH mechanic told me to try bending the fork so it made more contact with the gear. I did that and the tractor stayed in 5th gear from then on. Saved taking the transmission apart and replacing the 5th gear. So I would check the 5th gear fork if the gears look OK.

Show me a picture of the output on the center section and also how much slop is there on the colliar between the fork .
