Overriding coupler

I purchased a Hub City overriding coupler a few months back, it was what the 135 needed. Only problem it came disengaged from the tractor pto shaft, it and upper part of pto shaft was consumed by the sidewinder bushhog.
I have been using this item for 2 months and check it before each use. I have no Idea why it would come unlocked and fly off. Luckily the blades slung it out pretty quickly.
Hub City is sending me a replacement, they are baffled as I. All input apprecaited

How did you affix it to the pto shaft. You are supposed to use a roll pin. I wonder if you did, maybe the roll pin failed, due to maybe to loose a fit and sheared....Or something like that...

No idea why it would disengage from shaft. I purchased one several years ago from them for a Ford 8N and had no problems. Sold the 8N and now have a MF135. Glad to hear they gave you a replacement.
