Well, when I was still interested in (and able to afford) hot rods, they either used a vacuum pump as mentioned below, or a big ol' vacuum 'reservoir' that was topped up(or more properly 'evacuated')at closed throttle or whenever the intake showed a vacuum. After the first couple of weeks, you didn't spend ALL your time on boost ":^) Real 'lumpy' cams (long duration, high overlap, high lift) could cause the same problem, but this was primarily in track-only or barely streetable cars. Oddly, they didn't have a lot of vacuum-dependent accessories...
