Sounds from inside can be misleading. But if it really goes away in fifth it could be that the pilot bearing that connects the main drive shaft to the top transmission shaft is gone. In fifth the slider gear would hold the ends of the two shafts together. Or it could be something wrong with the main drive shaft pinion and the constant mesh gear that connect the main drive shaft with the transmission counter shaft. All the load is off those when in fifth, that could quiet it down. It might be that you could see or feel enough to check these by taking off the belt pulley or the cover where the belt pulley would be.
