In 5th gear, engine power goes straight to the differential (direct drive). In all other gears, power is transmitted through the countershaft. Sounds like you have a bearing problem on the countershaft. Unfortunately, this shaft is under the main shaft and is best viewed with the transmission cover off. You may be able to reach in through the belt pulley drive opening and check for looseness but you won't see a lot.
