Lock Washer AND/OR Locking Tab

I am reassembling the rod caps on a Farmall M. When I took the tractor down, it has spun a rod bearing insert. The cap screws had BOTH lock washer and a flat type washer with a tab that was bent up around the hex head of the cap screw. If anyone has a parts book for the M, can you advise me if both washers are needed. I have an idea that both washers may have caused the cap screws to loosen and caused the insert to spin. Thanks...
