PTO pops out of gear

I'm working on a 1939 H and the PTO pops out of gear.The slot the shaft engages into istorn up so it looks like a ramp on either side of it causing it to pop out under a load.Can someone e-mail me a breakdown of the transmission so I can see what I need to do.Is the shaft it couples to the main shaft or counter shaft and where can I get one.This could probably be welded and the slot recut.I'm sure this is somewhat common of a problemI imagine I need to split this thing apart to fix it right.Any advise is greatly appreciated,Brenden
