...real! I got one for each of my H's & Super C sometime in late Jan-came by USP. Used form from now defunct photo-point. Found a copy & #1 Son mailed it in for his H in early Feb. Told him to expect it to take a couple of months, as I mailed my form back in early Nov. Search archives, I think there might be one there that's not photo-point. (My $0.02 worth. jal-SD)
