Lots & Lots of sanding and sanding. Then a rust inhibitor and now you are ready to put on your first coat of filler type primer then sand and a second coat and then a third and then a forth, sanding between each coat, until you are happy with how smoooooth you have the surface. Then be sure to use the primer recomeded for the finish coat of paint.

Sand Blasting is not for the novice. It is really easy to cause the metal to heat to the point that it streches, just as Red Dave says.
