As far as I know it covers the letter series for sure, I think the number series came with the taller cap. no form is necessary: Send an email with your name, address, phone number, model serial number (if known)and model number (mandatory) for each tractor that needs a cap. To: Mark.Clark@Nav-International.com, or just call him (Mark Clark) at 260-461-1873 with the information. And here is the form if you prefer snail mail, right click, save, open it later and print it out.
