I would guess that you have a greater sensitivity to exhaust than most. I spent a good deal of time on tractors in my youth and the exhaust never bothered me unless there was a breeze blowing it directly on me. In general, since the exhaust is quite a bit hotter than ambient air, it will rise above you before it reaches the operator, but if you are especially sensitive to it there will probably be enough reaches you to cause problems. The extension is probably a good idea.
