If a noticeable groove has been worn in the thrust plate behind the cam gear, I'd say yes to replacement. Make sure the cam gear hub bottoms out on the shoulder of the camshaft. The thrust plate should be captured between the two but free to rotate. Could it be that the nut has come loose on the gear? It doesn't seem likely that the camshaft can drift that far back in the engine to hang up on a follower.

From the sound of it, 3/4 of a turn on the crank is only 3/8 of a turn on the cam. I suspect one of the combustion chambers has something in it. You didn't say whether or not the engine would turn over after getting the cam gear to turn. Since you've gone far enough to get the cam gear off, I recommend you pull the head and look carefully at all four cylinders.
