I agree. The belt needs to be VERY tight. Get a long pry bar to tighten between the 2 idler pulleys. Also try not to run the mower over ground that is not smooth-such as turning into a ditch with the mower all the way down. Engage and/or lower the mower after the tractor is straightened out. If the mower is twisted a little and the belt is only a little bit loose, it will come off. Also make sure the bearings in the pulleys are not worn.
