Jesse: When you remove lever as Alton has suggested, and then line the rails up. The end of you shift lever should be built up with weld until it just nicely fills the notch on one rail. The finished lever should have a rectangle shaped end very slightly rounded on corners. I like to get these just a bit over size and grind until it shifts smoothly. It takes a little time and patience. If you don't build it up it will just do the same again. If you happen to be shifting on the move and it stays in 4th and also goes into 3rd and you happen to drop clutch out, you will most certainly do damage. I had this happen once, result 4 gears with broken teeth and one bent shsft. Don't fool around with this.
