I agree with Rich, most likely your 4th and 5th speed sliding gear is worn out. Comes from grinding the gears when you shift on the go into 5th. Likely the teeth on the 5th gear part are mostly gone. I've seen this gear after market for around $100. From Case/IH it will be more.
