The sleeve has broken off at the flange, or it could not move. The head and head gasket hold the sleeves down to the block by compressing the flange in the block. Take the head off and find another set of sleeves and pistons, Just as well have the head rebuilt and put in new rod and main brgs., and you will have a new engine.
