it's possible that the problem is the pilot bearing between the trans. driving shaft and the main shaft. 4'th gear is right next to this bearing and will be noisy with failed bearing.It can be serviced from front of trans. by removing clutch shaft and bearing cage for trans. driving shaft. I would try this first, nothing lost as it will need to be removed to ser. 4'th gear. may save some labor of removing the top cover.
