Malfunction of the operator, you can't shift from 5th to 4th until the tractor is nearly stopped, by the time he found that out he was going backwards, and he couldn't shift into a forward gear while rolling backwards. Combine that with the typical unmaintained disk breaks and he is lucky to be alive -- all due to inexperience.
