What probably happened is that the shifter tip inside the shift rail system (and easy to get to) came uot of the forth gear slot and as you went into neutral to go to reverse it continued into fifth. I would make a little bit slower transitions into neutral and, if it persists, I would probably pull the shift lever, and get the tip of it welded back to size, then ground down to fit the slots in the rails. Align the slots with a big screw driver befor reassembly. A broken or weak spring in the rail detent system could also allow this to happen, but the platform has to be removed to get at these parts.
