twobit - I posted on that problem several years ago. I solved it by making up a spacer to go above the thrust bearing as you suggest. Unfortunately I did not record the dimensions of the spacer. However the dimensions are not terribly critical. Once you the pivot shaft disassembled it should be simple matter to determine a suitable spacer ID and OD. ID should be about .075 greater and OD .075 less than the thrust bearing. (Finished spacer should easily over the pivot shaft journal and freely into the bore in the fixed bolster casting) The spacer thickness will be distance you want the sector gear to ride lower in the steering pot - the sector gear should be centered on the horizontal worm. You can be off by .100 or more and it will work fine. Punch below for a few images of the overhaul on mine, including a shot of several different thickness spacers I turned.
