I had a Farmall M (same transmission as OS6)that would slip out of 4th gear unless you held it in. Gears were fine.
What I found was that not only was the detent notch on the shift fork worn, but the the hole in the transmission top cover where the spring loaded detent ball goes was egg shaped.
I got a good used shift fork and put a stronger spring in with the ball. To really do it right, I should have welded and re-drilled the egg shaped part of the detent ball hole in the top cover, but I didn't have access to a welder.
It was a bit harder to shift after that, but never slipped out of gear.
The part of your description where you said it slipped back 1/4" makes it sound to me like you have the same problem.
