If the wheel contacts axle on top of axle, (180 degrees from wedge) you may not be able to keep it from walking. The drive should come from the flats on axle only. Had to replace some wheels on new tractors but wheel that were originally good can wear and cause this problem also if run loose . I have sent dual hubs, we couldn't keep tight, to machine shops to gain clearance, never tried it with a wheel as they were under warranty. As pointed out, torque on bolt is important and you must retighten until they hold that torque. A little trick you can try is to slide a thin strip of steel on top of axle flat on each side and see if it will hold, would not necessarily recommend that as a permanent repair though.
