The first thing you need to be sure of is that the wheel contacts the flats on the axle and does not tighten on the rounded part. If it does not tighten on the flats you will never be able to keep it in place no matter how tight you get the wedge lock.
The other thing to do is to oil the wedge lock assembly, not only the threads but also where it contacts the wheel and the axle. It may sound counter intuitive but using a little oil will get the wheel much tighter with less torque on the wedge lock screw and they will stay in place.
