It would appear to me that when the wheels put pressure against the pipe spacers which in return would put pressure against the outer bearing cap that the spacers would cut into the spacers.
IMO, I would find out why the wheels slid in and ssolve that problem. I have been operating tractors since the mid '50s & the only time I have had a wheel slide is when it was loose.
I was taught that when you tightened wheel bolts that they need to be as tight as you can get them then turn them 2 more turns. WE always use a 4' foot pipe on a HD breaker bar. Tighten till tight then hit on top the axle with a sledgehammer then tighten again. Do this at 2 or 3 times.
Other possibility is that the wheel clamp bolts are bad & act tight but aren't. If you still have trouble with the wheels sliding after tighting like I said I would take the bolts out & check them.
