Get in the habit of making your shifts very deliberate and SQUARE. The biggest cause of getting stuck in two gears is shifting "diagonally" with a slightly worn shifter nub.
A couple of stern reprimands, and I think a rock heaved at my head, got me into the habit many many years ago :)
