I was ugly enough by the time I got it out that I never took it apart to see what it was made of -- whether a single piece in the middle threaded to take the bolt from each end, or two separate threaded parts embedded in the rubber. In any event, it's the rubber that has rotted/given out around the threads internal to it that are making your problem. Turning on the bolt head results in merely turning the whole rig around inside the rubber without ever acting on the threads.
If we're talking about the same thing and you're having the same problem, my solution was to grind off the old bolt heads and a judicious application of a prybar to free it all up to get it out of the way, and replace with new.
It's not anything you'll ever have to take apart again in your lifetime, but a sparing touch of an anti-seize compound on the threads in the replacement might be a good gesture toward the next guy.
