Coolant on the piston can be either a gasket (hopefully) or a cracked head. Either way taking the head off is necessary. If there is clear evidence of gasket failure, and no crack evidence, and I was a fast and cheap mechanic I would go with the gasket. If I was doing a valve job, or had a feeling it night be a crack, I would have the head checked for cracks. I assume the coolant is on the inside of the sleeve, and not between the sleeve and the block!! a different problem and not cheap. Tha usually requires a rebore of that cylinder and a repair sleeve, then bored to fit a new dry wear sleeve like the original. Jim
