Well first off you need to source the parts. This is one of those things you CAN'T get from the CaseIH dealer. It's got to come out of a salvage tractor, and then you need to know what you're looking for to make sure you don't just get another "regular" 4th gear set for an M.
4th gear on a Super M is the "fast" gear from the factory, so you may have better luck looking for a Super M, than a plain M with the somewhat rare 7MPH 4th gear option.
Then you need to split your M at the bell housing, remove the torque tube section, and completely disassemble the transmission. Swap out the gears, and it might be a good idea to replace some/all of the bearings while you're in there. Definitely new seals and gaskets. There's certainly some setup to do with shims to ensure proper gear mesh.
It's a job. That's why there aren't many Ms with the fast 4th gear out there.
