My dad had one of those spark plug air pumps. Kind of crude by todays standards, but it worked. We aired up a bunch of tires with it. Not too many farmers owned portable air compressors back in the 40s and 50s, and when the Farmall H was introduced in 1939, there were a lot of farms that didn't even have electricity yet, so that spark plug air pump was a very handy tool to have around.
And yes, the engine did run while it was being used, but it ran on only 3 cylinders because the air pump was occupying the hole for plug #4.
