Hard start

My 504 Diesel has a rough time starting. I warm up the glow plugs for about 10 sec. and hit the starter. She starts, but puffs smoke like hell,rpms decrease and she quits. By about the 3rd or 4th try the smoke clears up and she idles like normal.What gives? Could it be the fuel? I haven't put new fuel in it in about a year b/c i haven't used it enough to do so. I've been working on it since i got it in Aug. 2010. I replaced head gasket, glow plugs, filters, and all fluids except fuel shortly after i got it. only started smoking this spring.
