Something is not making too much sense here. The transmission input shaft, counter shaft, and reverse idler all turn in direct ratio to engine speed, not road speed. The transmission main (output) shaft, differential assemble, and final drive components all rotate at speeds directly related to road speed.
Normally when you have a complaint where there is noise in all gears except 5th the problem is in the constant mesh gears or lower shaft bearings. A bad pilot bearing noise would slow down as you moved to a faster gear.
About the only thing that I can think of that would match your problems is a noisy rear main shaft bearing where the race is bad on top. When in gears 1 - 4 there is a force pushing the main shaft up as the gears try to separate. When in 5th the force would only be to the side from the ring and pinion gears. What kind of noise do you have in reverse?
