Well, he had probably loaded and unloaded tractors 1000's of times in his 75 years. That's a job that really needs an experienced spotter, heck even two. "STOP!!!. Try again" is all that need be said.
I am guessing that his neck didn't rotate as well as it used to. He couldn't see his wheels as well as in earlier years. Sometimes you just need a younger man to do a job that is so familiar.
The same fatal accident happened to the owner of OEM Equipment several years ago. He was on a Cub Cadet while attempting to load it.
Good traction, adequate power at low rpms, and creeper gears make a big difference. Clutch feathering and improperly adjusted brakes can contribute to such accidents.
