I had this problem once, but mine was excessive end play. Someone had the PTO out at some point and dropped the thrust washer in the rear axle housing while removing it. The result was the PTO shaft moving back and forth during operation and working the clutch sleeve off the end of the countershaft. On a hunch, I purchased a new washer, but I didn't have the brains to check if the old one was still in there. It was. If your PTO is grinding while you're holding it engaged, you may have to change the clutch sleeve and hopefully not the countershaft.
