It is just something you learn by doing and you will learn a little bit every time you shift from 4th to 5th for awhile. I think I transitioned very easily as a kid as our farm truck was a 1950 Ford 3/4 ton 4 speed on the floor non-syno transmission which I learned first. On the M you will probably find it easier to shift slowly out of 4th while decreasing engine rpm's and slowly non-forceful feathering it into 5th. If you have some gear clash just try more time and different engine speeds and you will get it down fairly quickly is my guess.
