Probably the hydraulics would be one of the more costly problems. SMTA gas pump and manifold is different. Unless near end of MD production the front cover on the MD is different than a SMD through 450-D. MD starter won't work on the SMTA and a starter from a SMD stage 2 or SMTAD or late 6 diesels that works is not laying around everywhere. Then you need the linkage to engage starter and the switchover linkage is different. For a user a 400, 450 or some other diesel starters could be used with switch that would eliminate the need for linkage to starter. MD throttle rod would need lengthen and MD steering post a spacer under it. Just hit the high spots and not all odds and ends to complete. Things like MD hood, fuel tank, frame rails and other should be obvious.
