On harnessing the dog, I started with a collar and he could wiggle out of the best. He is a blond long haired Italian dog (saw his picture on the wall at the vet with a lot of other critters) and has a thick neck and smaller head.
I have him in a 40' square pen in the yard and take him out a couple of times a week. He gets so excited getting to get out and smell/you know on everything.
On your feeling better, stress is a killer. Worrying about your BP is stress and can kill you. I didn't realize how my body was reacting to the stresses of my job till I retired.
On your wife and your diets, my daddy contracted diabetes which ran in the family and he was a sugar lover and over weight, no exercise. Don't know which he had, type 1 or 2. Anyhew, mother controlled it with his diet, no insulin, and smacked his hand any time he tried to reach for a sugar item AND CHEAT!!!!! NO CHEATING, Grin.
