Bad waterproofing detail, they buried it for some reason. Masonry details for waterproofing are very important.
Whatever that brick is bearing on, foundation wall, lintel, etc. needs to be flashed, to the wall behind it in the cavity.
Look at details on the site I posted a link to, they are an industry standard in masonry accessories, waterproofing and so on, you need to design and build a detail that will waterproof that low threshold door, there must be a curb that it sits on above that slab, if not, really bad detail which will need to have remedial work to keep water out.
