I have a 92 K3500 pickup with the 454 / 7.4. It has 180,000 miles. The last 40,000 are mine.
It will rod knock for a few seconds at cold startup, if I dont run an oil filter with an anti drainback valve. One other possible thing is a clogged lifter gallery plug orfice. The big block Chevys had a cold start lifter noise problem from the first ones in 65. Chevy finally found that the oil would drain out of the lifter galleries and fill with air. On a cold start, air was pushed into the lifters, causing lifter noise untill the air worked it's way out and the lifters would be noisy untill they again filled with oil. Chevys fix was to replace the solid front lifter gallery plugs with ones with a tiny bleed hole drilled in them. This did two things, it allowed the air in the lifter galleries a quick way out, and it sprays a tiny stream of oil at the timing chain and sprocket to increase lube and chain life. It's rare, but on occasion, a bit of something plugges a gallery plug orfice and the cold start lifter noise comes back.
