People speed shift the ABCD ranges and grind them into gear. This wears the shift collars out. Then they start popping out of gear.
To check if it is the shift linkage shift it and "feel" for the detent. Don't just line it up with the shifter quadrant. If the detent and the quadrant do not line up then adjust the linkage until they do. If they do line up and you "feel" the detent an it pops out of gear then I would bet the shift collar is shot.
