like others have said a groove in the shaft and bad bearings are 2 of the most common reasons for seal failure, however there is another one, its improper installation, seals must be installed straight into the bore, some seals are double seals and these must be installed with a seal installation tool, pretty reasonable cost from h/f .im as guilty as anybody about installing a seal with a piece of wood and a hammer or using a socket of the appropriate size, but for double seals like found on places like truck wheel ends ect this will deform the seal, causing a instant leak, indeed not using a seal installation tool is risky on all seals, i found that out the hard way
