We had a first-year 6.9 F350 in our service truck at a Deere dealership. It never skipped a beat. A pig on fuel but very few problems with it.
I had a 1985 F250 with a 6.9. Got rid of it with 350,000 miles and still ran perfect. Never a single problem with it. 6.9 in a 4WD up against a C6 trans and 4.10 axles got me 9-10 MPG most of the time which was not exactly great mileage though.
When the 6.9 got bored out to make it a 7.3 - it was more prone to cylinder wall cavitation. Not what I'd call an overall "better" engine. Just different with a bit more power. I still have my last-year 1994 IDI turbo-7.3.
