Did you just pull the back shaft out when you replaced the PTO shaft??? I think you have to make sure the PTO shift collar is all the way forward which is disengaged IIRCC. This way the collar will stay in place. I think if you pull the rear shaft out with the shift collar back it can fall off.
Also if the rear PTO was broken did it break the shift collar too???? They both may have broken at the same time.
I think you can see the collar from the side if you take the cover off that the PTO engagement lever is on.
