this may sound odd but I worked for a place that had a 04 chevy with the same issues and after a new trans and a trip to the dealer they found out it was in the steering colum and the final cost for the colum was below $100. The total cost with a rebuilt trans and man hours almost scrapped the truck though. I also had a 94 that when it started that you would pull over shut off the truck pull out the key put it back in and restart go real easy and let it shift through the gears DO NOT run it hard until it gets into third and dont run it hard enough to cause it to down shift from third to second or you will to start all over again Hope this Helps John
