Well, buying any ten to twenty year-old vehicle is going to be a bit of a crapshoot, and decent used four wheel drive pickups are scarce. But GM pickups of that vintage are reasonably reliable. We bought a new 2004 Silverado 4WD, drove it for a couple of years and sold it to our son. He subsequently moved to Las Vegas for a few years, so the vehicle has spent half its life out west and half in Michigan, but it's still rust-free. It has been reasonably trouble-free, just the usual stuff: brakes and routine maintenance. He's had to replace a couple of bad injectors. Last week he got check engine light which turned out to be a knock sensor code. I did a little research and it turns out this is a common failure with the 5.3 V-8. The part is cheap but you have to pull the intake manifold to change it out.
Some years of these vehicles are known for getting a "piston slap", which sounds bad but doesn't seem to hurt anything. So knocking on start-up probably isn't something to worry about.
You might ask him why he needs a four-wheel drive pickup. Since he's a Marine, I assume he's either in Camp Pendleton, CA or Camp Lejeune, NC. Neither location is known for blizzards and snow drifts, so I have to assume he's thinking of going off-roading. If he takes his pickup out on the beach, it's not going to last long.

From the engine number, it was found that this Major was produced late 1946. It was almost complete but had various parts that would definitely need replacing.
