Usually they wear out, not often they break anymore.
But usually the symptoms are a "thud" you hear when accelerating or decelerating, or an annoying vibration from metal to metal contact when the engine or exhaust makes contact with the body or frame.
You can check them by opening the hood, put it in gear and hold the brake while bringing the power up, and watching the engine. Some movement is normal, but it shouldn't lurch upward.
If what you are describing is more of an engine surging, gaining and loosing power rapidly as the mounts flex, take a look at the rubber plenum between the throttle body and the air cleaner box. If it is loose or ripped down in one of the pleats, it will cause erratic engine surge. It leans the mixture because unmetered air is entering the intake.
