If you're handy, it's really not hard to make your own. My first cab I made out of 2x4's, plexiglass, and plywood for about $150. The biggest investment was the plexiglass, clear vinyl would have saved a bundle.
I think you'll find that heating isn't nearly as important as blocking the wind. Once you've blocked hte wind, the gradual heat from the engine and yourself will make the cab very tollerable. I spent 6+ hours in mine in 0-5F outside temps with nothing more than 2 pants, good boots, a down jacket, and a hat. Inside the cab was easily 20-30F above outside temps.
If you're water cooled, $150 for a cab heater off ebay, runs off a 5/8" hot water line.
