I'm sure there are tougher ones i forgot, but what comes to mind is the 6.4 powerstroke in trucks or the 6.0 powerstroke in vans, I think the labor guide is somewhere around 2.5 hours for either one and you'll earn it, this is just to replace the belt. Blood and cussing are guaranteed too.
Many front wheel drives are difficult too, removing motor mounts, supporting engine and cramming an inch wide belt in a half inch wide gap.
