Not sure of the rules, but I imagine that the trailer is never suppoded to disconnect in case of a rollover, so your hitch support must be able to rollover your truck, lift it, etc, with some acceleration to it (maybe 3G, so there times the weight).
Same goes for accidental braking. If you were to survive a front impact, you do not want your trailer to detach. Your connection needs to be strong enough to crush the front of the dodge by pushing on the ball hitch. It is obvious that 4x 1/2 inch are unsafe.
Some cross tubing welded to the plate would really help avoid flexing of the plate.
You need to have good contact between plate and chassis, or your bolts will fail . Bolts are failing due to repetitive excessive stress variations. Longer bolts mounted with solid spacers are better than short bolts.
2 piece of drilled sheet metal inserted between the bed ribs were the bolts are sticking to the chassis will really help: as you tighten, you want to elongate the bolt and have solid connection. You absolutely do not want to crush the bed ribs partially as you tighten. the ribs will continue to crush with loads, loosening your bolts.
Loose bolts have excessive stress variation everytime they are under load and broke quickly.
