When I was in training for DOT Inspection back 20 some years ago, the events you describe were more or less covered. It was recognized that you can't defy the laws of physics or the old "weakest link in the chain" thing. All you can do is your best to make sure things stay put. It's the idiots that use a 1" HF strap on an excavator bucket and laughingly call it "strapped" or just don't bother at all that make the rest of the guys doing their best look bad.
