A 200 amp disconnect in a high rise bus run was tripping . It had tripped the main . My co-worker tried to reset the breaker under load . I wasn't sure if he jumped or was thrown back by the charging handle. When the main reset the 200 amp disconnect several stories up exploded { it was concluded Square D had left an extra 1/4 twenty bolt between phases which eventually wore through the plastic coatings Which caused a phase to phase short. All the lawyers came running out of their offices saying when is power coming back Well my boss is on the phone with Square D in Ohio so they can start making a new piece of bus duct. Lawyers were not happy with my answer. The wall in the electric room looked like it was shot with a copper pellet shotgun . We were lucky we did not kill someone.
